T19: Feedback from Test Participants

This is a sample feedback form to be completed by the participants upon completion of a test.  It is important to survey the participants before they leave the test alternate (recovery) site.  Often, if feedback is not gathered just before the participants leave the test site, this collection of the feedback becomes a lengthy affair.

Thank you for your involvement in the Business Continuity Test. To help improve the effectiveness of future tests, we would like a few minutes of your time in answering several questions.

Your response will be kept confidential. Only consolidated results will be made known to management. Your feedback would be most appreciated.
