You should have downloaded the two forms from the certification application portal as directed by the Certification team. These include the Certification Application Form (CAF) and a Certification Application Checklist (CAF-CC) for submission to the Certification Review Committee.

Relevant certification awarded and conference/ seminar/ webinar attended during the last three years.
Business Continuity / Disaster Recovery / Crisis Management/ Crisis Communication/ Operational Resilience Conference(s) Attended

B. Course(s) / Conference(s)

Business Continuity / Disaster Recovery / Crisis Management/ Crisis Communication/ Operational Resilience conferences, seminars, and webinars attended for the last three years.

Submission of Application Form

Submit your completed Certification Application form and upload it to the certification application portal (CAP). You must ensure that the Application Form is signed (typing in electronically) and dated before submission.
