### Sustain Phase Appendices

The appendices bring the concepts discussed in **A Practitioner’s Guide to the “Sustain” Phase** into practical application.

The templates should be adapted to reflect the organisation’s size, complexity, regulatory environment, risk profile, governance structure, and operational resilience maturity.

#### Appendix S: Sustain Phase Maturity Assessment

**Overall Maturity Interpretation**

**Level 1 — Initial**  
Operational resilience is largely reactive.

**Level 2 — Developing**  
Core Sustain capabilities are emerging.

**Level 3 — Defined**  
Sustain processes are formally established.

**Level 4 — Managed**  
Performance is measured and actively governed.

**Level 5 — Optimised**  
Operational resilience is embedded into strategy and continuously improved.
