# \[OR\] View Certification Application Form: Section 1 & 2
You have downloaded the two forms from the certification application portal as directed by the Certification team.
These include the Certification Application form (CAF) and a checklist (CAF-CC) for submission to the Certification Review Committee.
What is the content of Sections 1 and 2 of the Certification Application form? This is an outline of the form that you have downloaded.
Note that access to downloading the CAF and CAF-CC forms from the Certification Application Portal will require you to complete payment of the certification processing fee.

You should have downloaded the two forms from the certification application portal as directed by the Certification team. These include the Certification Application Form (CAF) and a Certification Application Checklist (CAF-CC) for submission to the Certification Review Committee.
Note that access to downloading the CAF and CAF-CC forms from the Certification Application Portal will require you to complete payment of the certification processing fee.

- Relevant certification awarded and conference/ seminar/ webinar attended during the last three years.
