# View Certification Application "Completion Checklist" Section 1 and 2
You have downloaded the two forms from the certification application portal as directed by the Certification team.
These include the Certification Application Form (CAF) and a Certification Application Form Completion Checklist (CAF-CC) for submission to the Certification Review Committee.
What is the content of the Certification Completion Form Completion Checklist? This is an outline of the form that you have received.
Note that access to downloading the CAF and CAF-CC forms from the Certification Application Portal will require you to complete payment of the certification processing fee.

### Sections 1 & 2
#### Purpose of Form

The certification team directed you to download the two forms from the certification application portal (CAP).
These include the Certification Application form (CAF) and a Certification Application Form Completion Checklist (CAF-CC) for submission to the Certification Review Committee.
Note that access to downloading the CAF and CAF-CC forms from the Certification Application Portal will require you to complete payment of the certification processing fee.
This blog provides an overview of the checklist content for completing your Certification Application Form (CAF).
Please note that BCM Institute reserves the right to reject the application should there be missing submissions and requirements.

| | | **a** | One-year BC/DR/CM/CC/OR experience for BCCS or DRCS, CMCS, CCS or ORCS. | | |
| | | **b** | Three years BC/DR/CM/CC/OR experience for BCCE or DRCE or CMCE or CCCE or ORCE | | |
| | | **c** | One year of BC/DR or Audit experience for BCCA and BCCLA. | | |
| | **3** | | I have attached a Microsoft Word Application form to the application. | | |
| | **4** | | I have re-name my application form: YYYYMMDD (e.g., 20240102)-Certification Name (e.g., BCCE)-Name of Applicant (e.g., Name on certificate) V 1.0 (Version) | | |
| | **5** | | The application process will take 4 to 8 weeks from the first or revised submission date. | | |
| | **6** | | I have uploaded the certification application to the certification application portal. | | |

| Submit your completed Certification Application form and checklist by uploading them to the certification application portal.
