Explanation

This template is used as a detailed follow-up worksheet for exceptions that require attention.  The item is extracted from the form “Template T14: Record of Exceptions Noted During Test.”  This template details the problem and solution as well as tracks the completion of the change (i.e. change has been done and plan has been updated).  This is vetted by the Business Unit Coordinator or his/her immediate supervisor.
