Request for Information for HBP Town Council

This is an initial list of request for information prior to the start of the WSQ training -led consulting project.

Information on Business Units Participating in Project

  • List of Business Units involved in the Business Continuity Project
  • List of Business Unit Coordinators for each Business Unit
  • List of Vice Business Unit Coordinators in the event of the absence of the main Business Unit Coordinator
  • List of Head of Business Units
  • Contact details (Handphone, email) of each of the individuals above (develop Call Tree)
  • General Business Functions by Business Unit
  • Critical Business Functions (Functions that HBP cannot do without for an extended period of time)
  • List of IT software/applications used by HBP in their day-to-day operations
  • List of vital records/important documents that must be retrieved in the event of a disruption at a HBP site
  • HBP’s Minimum Business Continuity Objectives (MBCO) also known as the bare minimum that HBP Town Council needs to keep running even in the event of a disruption

Scope of the project

  • List all locations (with Business Units) involved in the HBP BCM Project
  • List of all alternate sites (existing or planned) that HBP staff can move towards to resume work in the event of a disruption. If possible, please provide a layout of the alternate site.
  • Command Centre(s) used by HBP’s senior management board to gather and make decisions in the event of a disruption/crisis. If possible, please
  • Assembly area for HBP staff in the event of a disruption
  • Alternate Assembly area in the event of unavailability or wet weather

Crisis Teams

  • Organizational structure of HBP Town Council
  • Crisis Management Team composition (inclusive of member names and designation)
  • Business Continuity Team (Inclusive of the organizational BCM Coordinator and anyone else assisting him)

Existing Plans

  • Any existing emergency response or crisis management or pandemic flu plans
